* Strategies to Prevent Unsafe Opioid Prescribing in Primary Care among Patients with Acute or Chronic Non-cancer Pain ($5 Million/3 Yrs)

* Community-based Palliative Care Delivery for Adult Patients with Advanced Illnesses and their Caregivers Advance Care Planning (up to $10M over 5 Yrs).

*Management of Care Transitions for Emerging Adults with Sickle Cell Disease ($8 M/5 Yrs).

* Foreign Organizations may apply if there is demonstrable benefit to the US healthcare system and US efforts in the area of patient-centered research.  A US partner may also be beneficial.
